HC was awarded a Resilient Communities Fund grant from the Ontario Trillium Foundation. Over the next 12 months, the grant will help HC re-imagine its programs and service delivery by developing online support, social, educational and health-focused initiatives for its members. To do that, we need your help.

We would like to know how we can best support your needs, now and in the post-pandemic future. Please complete either our COMMUNITY SURVEY or the MEDICAL & ALLIED PROFESSIONALS SURVEY. These confidential and anonymous surveys, give you the opportunity to tell Hydrocephalus Canada what matters to you! All survey participants are eligible to have their names entered into a draw for a chance to win one of three Amazon Gift Cards valued at $100. The survey will run August 23-September 24 and all contest entries, whether sent online or by mail are due by September 24 at 11:59 p.m.
Hydrocephalus Canada will use your feedback to improve existing programs, launch new programs and help identify which activities are most desired in virtual and in-person settings. Learn more about our privacy policy and click one of the buttons below to take our survey!

Community Needs Survey 2021 Contest

GENERAL RULES AND REGULATIONS
1. Eligibility
This Contest is open to legal residents of Canada who are over eighteen (18) years of age. VOID WHERE PROHIBITED BY LAW. The following individuals are not permitted to win the Hydrocephalus Canada Community Needs Survey 2021 Contest:
2. Eligibility

The Entrant must complete the Hydrocephalus Canada Community Needs Survey 2021 by the deadline date of September 20, 2021 at 11:59 p.m. to be eligible to win. By entering the Contest, the Entrant consents to the release of his/her name in the event they are selected to be one of the winners. The Entrant declares to have complied with the Contest Rules. Further, the winners must conform to all directions given by the Contest organizers or his/her participation in the Contest will be denied.

3. Contest Period
All registrants must have submitted a completed Hydrocephalus Canada Community Needs Survey 2021 by September 24, 2021 at 11:59 PM EST. Only one survey can be submitted per individual. Surveys and contest entries received by mail will be accepted until September 24, 2021 at 11:59 p.m.
4. Prizes
There are three prizes to be awarded. The prizes are three (3) individual Amazon Gift Cards valued at $100.00. These prizes are not refundable nor transferable and cannot be extended. Documents once issued in the name of the winner are not transferable or refundable. Prizes must be accepted as awarded and no cash equivalent is available. NO SUBSTITUTION OF PRIZES IS PERMITTED EXCEPT BY HYDROCEPHALUS CANADA. HYDROCEPHALUS CANADA RESERVES THE RIGHT TO SUBSTITUTE ANY PRIZES WITH ANOTHER PRIZE OF EQUAL OR GREATER VALUE.
5. Winner Selection and Qualification
A random draw will be executed by Hydrocephalus Canada for the three (3) prizes from all eligible entries received during the Contest period (August 23-September 24, 2021 up to 11:59 p.m.). The odds of winning a prize will depend upon the total number of eligible entries received on or before the applicable Contest closing date. Prize winners will be announced within 72 hours of the contest closing date, and notified by Hydrocephalus Canada directly by phone and/or email. Prize winners will also be announced via Hydrocephalus Canada’s social media channels (@hydrocephaluscanada on Instagram and www.facebook.com/HydrocephalusCanada on Facebook, @MY_BRAINWAVES on Twitter). Prize winners will also be posted on Hydrocephalus Canada’s webpage at mybrainwaves.ca/2021-hc-community-needs-survey/. By entering this Contest, the entrants automatically agree to accept and abide by these General Rules and Regulations.
6. General Terms and Conditions
Contest entrants agree to abide by the terms of these General Rules and Regulations. This Contest is governed by the laws of Canada. All federal and local laws and regulations apply. Return of any prize/prize notification as undeliverable may result in disqualification and selection of an alternate winner. By entering, participants release and hold harmless the Contest Sponsors Hydrocephalus Canada and their respective parents, subsidiaries, affiliates, directors, officers, and employees (collectively, “Contest Entities”) from any and all liability for any injuries, loss or damage of any kind arising from or in connection with the Contest, or the use or acceptance of any prize won. Restrictions, conditions, and limitations apply. BY ENTERING THIS CONTEST, YOU AGREE TO BE BOUND BY THESE GENERAL RULES AND REGULATIONS. The rules will be posted at all times at mybrainwaves.ca/2021-hc-community-needs-survey/.
7. Limitations of Liability
If, for any reason, the Contest is not capable of running as planned, Hydrocephalus Canada reserves the right at its sole discretion to cancel, terminate, modify, or suspend the Contest. Except as expressly warranted herein, the Prizes are provided to winners as is without further warranty of any kind.
IN NO EVENT SHALL CONTEST ENTITIES BE RESPONSIBLE HEREUNDER FOR ANY INCIDENTAL, CONSEQUENTIAL, SPECIAL, OR INDIRECT DAMAGES, EVEN IF CONTEST ENTITIES HAVE BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES.
